Before you apply, make sure you have the following ready. Documents vary depending on your vehicle type.
For ALL applicants:
Valid Government-Issued ID (at least one)
Driver's License
LTO Registration of your vehicle
Vehicle Insurance
NBI Clearance or Police Clearance
For OK! Ride (4-wheel) partners:
TNVS Certificate, PA (Primary Authority), or CPC (Certificate of Public Convenience)
Truck for Hire Permit — required if using your 4-wheel vehicle for OK! Express Delivery
Additional requirements based on your situation:
If your vehicle is NOT registered under your name: Authorization Letter / Special Power of Attorney (SPA) from the registered owner
If you are 51 years old or older: Medical Certificate from a licensed physician
